Per the Departments Scope of Practice, this position requires providing services to Therapy Patients, ages between neonates to geriatric in a manner that demonstrates an understanding of the functional/developmental age of the individuals served. It is expected that the associate demonstrate behavior consistent with the Core Values. The associate shall support the CHRISTUS St. Frances Cabrini’s strategic plan and the goals and direction of their Performance Improvement Plan (PIP) Major Responsibilities: •I dentifies and visits patients who have frequently used the emergency department for non-emergent episodes. • Obtains reliable patient demographics and contact information from patients in the emergency department. • Conducts interviews with patients to begin an assessment of the barriers and circumstances that led the patient to the emergency department. • Addresses and closes the gaps in service coordination. • Provides patient with information and linkage with a primary care provider if patient without provider. • Contacts patient’s primary care provider and advises of ED visit. Schedules follow-up appointment for patient with provider. • Provides patients with information and linkages to health and social support services and direct referrals to the Community-based Community Health Worker for continued follow-up and assistance in the community. • Contacts patient and referral sources within 24 hours after discharge from the emergency department to assure service coordination and continuity of care. • Educates patients and families about medical homes and the proper use of the emergency department.
